- 1). Begin the form with your company information or logo. Include the name, address and phone number. You can also include a business email address.
- 2). Skip one or two lines and create a section titled "Proposal." Create several blank lines in this section so you can write in the specifics of the work to be done when you are ready to complete a contract with a client. These lines will be used to indicate if the contract is based on weekly, monthly or on-call service.
- 3). Create lines near the bottom to input the date the work will begin and the date it is expected to end.
- 4). Create signature lines at the bottom. Put one signature to the left margin and one to the right for the client's signature. Use this form as the template to take to your printer to show how you want your customized contract to look. Indicate to the printer whether you want the form done in duplicate or triplicate sets. You can print your own copies from your computer and make a copy of the contract for the purchaser once it has been signed.
