- 1). Cut a white piece of 8-by-10 inch card stock paper into fours with a pair of scissors. You should have four, 4-by-5 1/2 inch pieces of paper at this point.
- 2). Color both sides of each of the pieces of paper with a neon red or neon yellow marker.
- 3). Line the border of the piece of paper with a 1/4-inch wide tipped black marker.
- 4). Write a word or phrase on the tag using the black marker to explain why someone shouldn't use the machine or piece of equipment. Write each letter in capital letters. Suggested titles include "Caution," "Out of Order," "Out of Service," or "Do Not Use."
- 5). Punch a hole through the center of one end of the card with a hole punch about 1-inch in from the edge. Stick a polyvinyl paper hole reinforcement ring on each side of the punched hole.
- 6). Thread a piece of wire through the hole and then wire the homemade lockout tag on the piece of equipment or vehicle in an easily noticeable location.
