- 1). Apply for Social Security Disability benefits. To do so, complete and submit a Disability Benefit Application, an Adult Disability Report and an Authorization to Disclose Information to the Social Security Administration form to the Social Security Administration. You can obtain a copy of these forms online at the official website of the U.S. Social Security Administration (see Resources) or by visiting your local Social Security office (see Resources).
- 2). Wait three to five months for a decision on your application. The Social Security Administration evaluates your application during this time period. When it has made a decision, it sends you a letter indicating whether your application has been approved or denied.
- 3). Go to the Social Security Administration's website if you don't receive a letter within the specified time period. Click on "Check the Status of Your Application" (see Resources).
- 4). Enter your Social Security Number and the confirmation number that you received when you applied for disability benefits. You're taken to a page that indicates whether your application has been approved, is still pending or has been denied.
- 5). Visit your local Social Security office to inquire about whether your application has been approved if you don't have online access. You may also telephone the Social Security Administration at the phone number listed on its website.
