- 1). Get the tax forms you need from the IRS to report your childbirth education income. Most post offices have tax forms at tax time, as do public libraries. If you use tax preparation software, all of the forms you need are usually included. If you received a W-2, you can file Form 1040, 1040-A or 1040-EZ. If you received a 1099, you'll need to file a Schedule C or Schedule C-EZ for self employment in addition to your 1040.
- 2). Report the type of business in item B on the Schedule C. Check pages C-9 to C-11 of the Schedule C Instructions and pick the choice that most closely describes your business. For example, if you were a childbirth educator in a hospital setting, you might choose 622000 for "Health Care & Social Assistance: Hospitals."
- 3). Fill out the tax forms with the amounts reported on your W-2 or 1099. If you received a 1099, fill the amount in Line 12 of Schedule C or Schedule C-EZ. If you received a W-2, report your income as a childbirth educator on Form 1040, line 7.
- 4). File your taxes as normal. If you filled out Schedule C, make sure to include it with your 1040.
