- 1). Download and print the complaint form from the Missouri Office of Chief Disciplinary Counsel website, (mochiefcounsel.org). Complaints made to the Missouri Office of Chief Disciplinary Counsel must be made in writing.
- 2). Fill out the complaint form by providing name, address and telephone number. Provide the name, address and telephone number of the attorney whom the complaint is against. If you or a member of your family has previously filed a complaint against the attorney, indicate the date it was filed and disposition. Indicate your relationship to the attorney, when the attorney was retained and amount of money paid to the attorney. If the complaint is regarding a specific lawsuit, provide the court where it was filed, the title of the lawsuit, the case number, date of filing and your connection to the lawsuit.
- 3). Draft a detailed statement of the complaint. Provide a complete factual summary, including names and dates of what the attorney did or did not do in the case. Sign the detailed statement.
- 4). Make copies of relevant documentation, which can include a fee or retainer agreement, canceled checks, pleadings, legal filings or correspondence between you and your attorney.
- 5). Mail the complaint form and supporting documentation to the Missouri Office of Chief Disciplinary Counsel (OCDC; 3335 American Avenue; Jefferson City, MO 65109). There is no cost to file a bar complaint in Missouri. After filing, an attorney with the Missouri Office of Chief Disciplinary Counsel will review the complaint and determine if it will be investigated. If the complaint is investigated, the attorney will be given an opportunity to respond to the complaint, to which the original complainant can respond.
