- 1). Obtain the contact information for your state's nursing board. This information is available online. You can also get it from the hospital or care organization you are working with.
- 2). Write a letter to the state board requesting the information. Be sure to include the nurse's name and what hospital she works for. You can also use this information to file a complaint if you wish.
- 3). Access the information online. Many state boards provide nurse's disciplinary information in an online database. If you cannot access the information directly, then you may be able to submit your request online.
