- 1). Obtain form SS-5 from the Social Security Administration (SSA) either online or at any SSA office. This is the form to fill out for a replacement Social Security card.
- 2). Enter the required information on form SS-5. Include your full name, full birth name, birth date, Social Security number, place of birth, citizenship, ethnicity, race, sex and the names and Social Security numbers of the parents listed on your birth certificate. Provide your address and sign and date the application.
- 3). Deliver or mail your application along with proof of your identity and proof of citizenship to your local Social Security Administration office. Acceptable forms of identity include a driver's license, state-issued ID card or a passport. To prove your citizenship you must have a birth certificate, naturalization certificate, certificate of citizenship, passport or consular report of birth abroad. Your new card is mailed to you when your application is processed.
