- To receive a replacement Social Security card, you must be able to provide proof of your age. Acceptable proofs of age include a birth certificate or a U.S. hospital record stating your date of birth, a religious document received before age 5 such as a baptismal certificate, a final adoption decree or a valid U.S. passport.
- To prove your identity, you must present a document showing your name, date of birth and a physical description or photograph. Acceptable documents required as proof of identity include a driver's license, a state issued non-driver identification card or a passport.
- If your citizenship is not on file with the SSA, you must prove your United States citizenship to receive a new Social Security card. Proof of citizenship can be a birth certificate, passport, a consular report of birth or a certificate of citizenship or naturalization.
- If you need a replacement Social Security card and you are not a citizen of the United States, you must come prepared with paperwork from the Department of Homeland Security. Acceptable information includes a valid visa that can be read by a machine, a valid foreign passport or a United States work permit card.
