- 1). Review the Adult Disability Checklist, available at the SSA's official website (see Resources). Collect the listed necessary documents and information that you need for the application process.
- 2). Visit the SSA's Benefit Application website to begin applying. Browse the guides listed for information about applications for the visually impaired, Medicare and other related topics. When you are ready to apply, check the appropriate boxes to indicate whether you are applying for yourself or another person. Read the Privacy Act Statement on the page and check the box to indicate that you have done so. Click "Apply for Benefits" and follow the onscreen instructions on each page to complete the application.
- 3). Visit the SSA's Adult Disability Report page. Read the Disability Information guides to learn more about how the SSA responds to applicants for disability benefits. When you are ready to file your report, check the box indicating that you have already read the Privacy Act Statement. Click "Start Report" and follow the instructions given on each subsequent page. At the end of the report, you will be prompted to print a page for your records.
- 4). Download the SSA's Authorization to Disclose Information sheet (see Resources). Print the pages and read over the information included on the second page. Complete the form and submit it to the SSA. You can mail the form or deliver it yourself to the local Cleveland office.
- 1). Call the SSA's toll-free number (1-800-772-1213).
- 2). Follow the instructions on the automated system until you are speaking to an SSA representative.
- 3). Explain your request to the representative on the line and tell him or her that you would like to schedule an appointment at your local office in Cleveland. You will have to give a limited amount of personal information to make the appointment, but the actual application process will take place at the Cleveland office during your interview.
- 4). Wait four to six weeks for a notification as to whether your application has been accepted or denied. If disability benefits are denied, you can reapply using the Internet Appeal form on the SSA's website (see Resources).
