Which is the Best Aromatase Inhibitor
Since the discovery that aminoglutethimide inhibited steroid synthesis and reduced estrogen production, aromatase inhibitors have been used for the second-line treatment of postmenopausal breast cancer.

The development of more potent selective aromatase inhibitors has provided greater clinical efficacy and improved tolerability. Short term adverse effects are typically mild to moderate and seldom require therapy to be discontinued. However, the long term effects of estrogen deprivation are unknown and require further study.

The role of aromatase inhibitors in the treatment of advanced breast cancer is well accepted. Their use in adjuvant therapy of breast cancer is under investigation.

There appear to be differences between the efficacy and tolerability of aromatase inhibitors (see Differential features table). Anastrozole and letrozole seem to have relatively greater efficacy than the other agents in the class. However, the lack of direct comparisons make it difficult to conclusively differentiate between the drugs.
