- 1). Request a DBE application by going online to the Wyoming Department of Transportation’s website (see References). Click on the link entitled “Request for DBE application”.
- 2). Fill in all blank spaces on the application request form. The areas with red asterisks require a response, otherwise, the application will not submit.
- 3). Complete the DBE application after you receive it by mail. Mail the application, along with a notarized statement of your personal net worth and documentation of your current Small Business Administration 8(a) certification, to the Wyoming Department of Transportation DBE Program. Mail all documents to:
